- Disbursement Form (Green Form) - available as Adobe PDF . Fill this out if
you have an invoice to pay - or you need a reimbursement for something
you've purchased. Please be sure to include receipts that shows the
item(s) and the price(s).
- Deposit Form (Pink Form) - available as Adobe PDF. Complete this form when
you have checks to submit for deposit. Depending on the event, some
deposits will go to the general account and some go to the student
account (and in some cases, they go to both). Be sure to include the
budget category (snack bar sales, Subway sales, etc.) and the amount
that goes to each account - general or student.
- Cash Deposit Form (Blue Form) - available as Adobe PDF. If you are chairing a fund
raiser that involves primarily cash for deposit, please use the "Cash"
Deposit Form. This will enable you to tally up the currency you have,
and serves as a better way to manage the actual amounts of money you
submit for deposit. The form can also tally checks if your deposit
includes both checks and cash.
- Spreadsheet for Totaling Multiple Checks - available as Microsoft
Excel. A simple Excel spreadsheet that you can use when you have
multiple checks to deposit. If you enter the check number and the
amount, it will "do the math" and give you the total amount for
deposit. Please send a copy of this spreadsheet (either hard copy or
electronically as an attachment).
- Fundraiser Financial Summary (Yellow Form) - available as Adobe PDF.When your event is over -
and all bills have been paid and money deposited, please complete this
financial summary form. We use it to track the total income, total
expense, and "net profit" for the event. We also use it to show how
much of the net from that event goes to the general fund, and how much
goes to the student account fund.
